If you ever heated it up or cooled it down too rapidly, that could have caused hairline cracks responsible for breaking the whole dish somewhere down the line, weeks or even months later.

I have several porcelain casserole dishes, which I prefer, because they can also go under the broiler- which is so helpful with certain dishes.
If you have your eye on "pyrex" soda-lime glass, I wouldn't go that route.
